Best way out of this is to trick it into thinking it has a DEF failure active, easiest way to do this is to unplug the DEF doser and run it for a bit (partially though a regen works), plug in the DEF doser, cycle key for a full power down, start it up and let it idle. It should idle, clear DEF doser fault then disable the derate.
